Lodgement Notes
Purpose:
This form provides for a natural person to alter his or her name.
One document should be used for each proprietor's name to be changed regardless of the number of titles or estates/interests affected. Should a document be prepared disclosing multiple applicants requesting their names to be changed (eg 3 separate proprietors on the one title) then it will be assigned one number per applicant and charged one registration fee per number.
Change of Name document is not required as a result of an error made by the LTO or if the registered proprietor is entering into a dealing disposing of their entire interest.
The A4 panel form, in part, takes the form of a Statutory Declaration, which must be signed in the presence of an Authorised Functionary (in South Australia, a Justice of the Peace, Notary Public, Commissioner for Taking Affidavits in the Supreme Court of South Australia, Proclaimed Member of the Police Force, or a Proclaimed Manager)
