Purpose:
The application is to be used to alter a company's name where the name has been registered incorrectly or where the company has changed its name.
One document should be used for each name to be changed regardless of the number of titles or estates/interests affected. Should a document be prepared disclosing multiple applicants requesting their names to be changed (eg 3 companies disclosed as registered proprietor on the one title) then it will be assigned one number per applicant and charged one registration fee per number.
Change of Name document is not required as a result of an error made by the LTO or if the body corporate is entering into a dealing disposing of their entire interest.
