Alteration/Change of Name (Subsidiary Interest)

Purpose:
This application is used to note the alteration/change of name for a natural person or body corporate for a Subsidiary interest such as a Mortgagee or Lessee.

One document should be used for each proprietor's name to be changed regardless of the number of titles or estates/interests affected. Should a document be prepared disclosing multiple applicants requesting their names to be changed (eg 3 companies disclosed on the one mortgage) then it will be assigned one number per applicant and charged one registration fee per number.

Change of Name document is not required as a result of an error made by LTO or if proprietor is entering into a dealing disposing of their entire interest.

The A4 panel form, in part, takes the form of a Statutory Declaration, which must be signed in the presence of an Authorised Functionary (in South Australia, a Justice of the Peace, Notary Public, Commissioner for Taking Affidavits in the Supreme Court of South Australia, Proclaimed Member of the Police Force, or a Proclaimed Manager).
